(either at ST3 or into higher years if previously in a locum
appointment for training). Current trainees will start the new
curriculum when they next enter a new training level (for
example, into ST4 from ST3), even if the ARCP for their
current training level fell shortly before this date, and similarly,
even if they do not move training placement (i.e. hospital/trust).
However, trainees will not transition to the new
curriculum when moving between training placements prior
to their training level transition (for example, a trainee moving
between trusts in August would stay on the 2016 curriculum,
but would start the 2021 curriculum when they
transitioned training levels from ST4 to ST5 in February). In
the same way, less-than-full-time trainees do not arbritarily
transition with the chronological (yearly) ARCP, but on
transitioning between training levels (for example, from
ST4 to ST5).
All trainees must have transitioned to the new curriculum
by 2 August 2023. The only trainees not required to transition
are those currently in, or entering, ST7 as their next
training level (i.e. current ST6-7s), who may choose which
curriculum to complete ST7 on. As an example, this would
enable a future pelvic oncologist to avoid chasing percutaneous
nephrolithotomies, but in order to do so, the trainee
would need to be able to demonstrate a completed special
interest module and all core competencies, including the
required number of IPs and an MCR documenting the
appropriate level of GPCs and CiPs.
Finally, the ST4 waypoint will no longer be enforced,
as the new checkpoint will be at ST5. Going forward, the
ST5 checkpoint will determine the eligibility of trainees
to sit the Intercollegiate Specialty Exam in Urology
(FRCS Urol).
Conclusion
While change can be daunting, the 2021 urology curriculum
can be viewed positively as a move away from the
arbitrary collection of records of WBAs and exposure to
INs, and towards the holistic professional judgements of
competency that are more in keeping with lifelong consultant
practice.
