If you have bunions or hammer toes, it's
worth seeking treatment, as one surgical
procedure could make all the difference

T

here is a range of corrective
surgery available for those
with deformities of the
forefoot including bunions
and hammer toes, as performed by
specialist foot and ankle surgeon,
Mr Abhijit Guha. Here, he explains
the techniques which he uses to help
patients who have such deformities.
'For hallux valgus (bunion) correction,
I use Scarf and Akin procedures, which -
in terms of correction - provide versatility
and reliability in even severe deformities,' says
Mr Guha (pictured below). 'Immediately after
the procedure, patients are weight-bearing
on the operated foot in special shoes
which we provide them with. Six weeks
after surgery, patients are allowed to walk
in normal, comfortable shoes again.
'For correction of hammer toe and claw toe
deformities, surgery incorporates soft tissue
releases and realignment, with bony
procedures as necessary. I use the latest toe
implants to hold toes straight instead of relying

on wires sticking out of the ends of toes. This
ensures an early return to both normal and
sporting activity following toe surgery.'
With longstanding complex deformites of the

consultant in trauma and orthopaedics
at the Worcestershire Acute Hospitals

utilising fusion procedures between various
joints. Following such surgery, the foot may not
be used for weight-bearing for the first six
weeks and, thereafter, walking is permitted only
in a protective and supportive boot which may
be taken off at night. Mr Guha uses modern
arthroscopic (keyhole) surgery for most of his
fusion procedures - this ensures early healing
and quicker rehabilitation by preserving the soft
tissue envelope around the joints involved.

Advanced techniques

Mr Guha treats various sporting injuries around
the foot and ankle using the latest technological
advances to get patients back into their field
of action. 'In ligament injuries of the ankle,
I augment anatomical repair of the ligaments
with the "internal brace" device to provide
early strength and aid quicker rehabilitation.
'I use dissolvable sutures for the skin, which
eliminates the discomfort of stitch removal,'
he continues. 'Surgery on the foot and ankle is
usually undertaken under a general anaesthetic
supplemented with a nerve block for post-

operative pain relief. This ensures early
rehabilitation and easier early mobility.'
Following surgery, patients will receive
individualised care and attention from Mr
Guha. 'I provide a complete holistic package
of care with physiotherapy and orthotic support
as required, to get patients back to their
normal level of activity as soon as possible.'
