Mr Fahad G Attar
FRCS (Tr&Orth)
is a consultant
orthopaedic
knee surgeon
Mr Attar is based in Cheshire in the
north west and covers both Manchester
and Liverpool areas in his practice. He
has trained in world-renowned centres
including Sunnybrook hospital in
Toronto, Canada and Oxford, England.
His practice and expertise encompasses
all aspects of knee surgery and he has
a special interest in managing young
adult knee arthritis, joint preservation
surgery and patient-specific knee
replacement procedures. Mr Attar is
also well published and has been invited
to present in meetings worldwide.

The initial management of knee arthritis
pain is conservative and involves:
*Activity modification
*Weight loss
*Physical therapy
*Regular painkillers
Further management options in people
where conservative methods have failed have
previously proceeded to joint replacement
surgery. This is a suitable option in some
instances, however as medical professionals
are currently seeing an increasing number
of physiologically and chronologically
young people with arthritis, there is now a
big push towards joint preservation surgery.
There have been numerous advances
in recent years in surgical modalities for
managing and addressing knee pain due
to various causes and these new joint
preservation developments aim to improve
both function and pain, while at the same
time delay the need for more aggressive
joint replacement procedures.
Joint preservation surgery has come to
the forefront recently due to the fact that
knee replacements invariably wear out
and the patient will require further invasive
revision knee replacement procedures which
are a big undertaking. Furthermore, routine
knee replacements currently have a 15-20%
dissatisfaction rate in reports worldwide.
So, in delaying the initial joint replacement

need to get to a stage where a revision

WHAT'S OUT Suffering with pain following
a meniscectomy (cartilage excision) and
waiting for the inevitable arthritis to set in
before any treatment is started.
WHAT'S IN Meniscal replacement surgery
is for people who have had cartilage tears in
the past - and have had their cartilage removed
partially or totally through keyhole surgery -but
now have pain in that aspect of their knee.
This pain is due to overloading the joint lining
in the knee causing some early wear in the joint
because the protective function of the meniscus
is lost due to previous cartilage resection.
Meniscal replacement procedures help by
reducing pain, improving function and at the
same time protect the joint lining in the long-run
by delaying the onset of further wear and arthritis.

localised worn areas in the knee, providing
pain relief and improved function and so
invariably avoid or delay more invasive

WHAT'S IN Patient-specific bespoke knee
replacements to address the arthritis in your
knee. These implants preserve more bone and
are designed and made specifically to match
your knee anatomy. These have shown to aid
in faster recovery and reduced hospital stay

and further hope to address the 15-20%
dissatisfaction and pain rates medical experts
still tend to see following routine knee
replacement procedures. There are also patient
specific partial knee replacement options.
WHAT'S OUT Three to four hospital
appointments over two to three weeks for
consultations, investigations, follow-ups,
physiotherapy and eventually treatment!
WHAT'S IN A one-stop knee clinic. This
service streamlines the care package to provide
a comprehensive service and takes around
two to three hours in just one hospital visit to
address the specific knee complaint. This
includes the first consultation, any investigations
required such as X-rays and MRI scans and the
follow-up appointment with the results of these
investigations which are reported by a
radiologist. Furthermore, a physiotherapist
review is arranged if required and some
treatment modalities started depending on the

(keyhole surgery) is required, this can also
